Before You StartKnow what you need to collect and what should happen after submission: which workflow should trigger, which tags should apply, which page should load.
Forms are your primary mechanism for collecting information and converting visitors into contacts. Every form submission creates or updates a contact record and can trigger any automation in your account.
Steps

Go to Forms
Navigate to Sites → Forms. Click New Form. Give it a descriptive internal name.

Add fields
Click Add Element to add fields: Text, Email, Phone, Dropdown, Checkbox, Radio buttons, Date, File upload, or Hidden fields. Every form should include at minimum an Email field. Map each field to a CRM contact field.

Set field validation
Mark fields as Required where necessary. Add placeholder text to guide users. Keep required fields to the minimum needed.

Configure the submit action
Set what happens when the form is submitted: redirect to a thank-you page, show a confirmation message, or trigger a specific workflow.

Save and get the link or embed code
Save the form. Share it as a direct link, embed on a MONDAi page using the Form element, or embed on an external site using the embed code.

Test the submission
Submit the form with test data. Verify the contact is created in your CRM, tags are applied, and any configured workflow triggers.
Common Mistakes
- No email field. Without email, the submission cannot create a usable contact. Email is the minimum required field for any lead capture form.
- Not mapping fields to CRM contact fields. Form data that is not mapped only exists in the form submissions view and cannot be used in automations or personalisation.
- No submit action configured. A form that submits to a blank page wastes the momentum of the person's intent. Always configure a meaningful submit action.
